What I have? 700r4/3.42 gears/4wd/33" tires/2.5" lift
Here are my issues:
1. tranny constantly shifts at even the slightest inclines driving down the highway.
2. tranny hits hard from 1st to 2nd...KACHUNG! let off on throttle just prior to shift and it lightens up a bit.
3. 4th gear...what's 4th gear!?! I have to be climbing a mountain with my foot in the engine to get it to shift to 4th and it hits hard too.
TV cable adjusted, oil/filter changed, stuck accumulator fixed...all done by certified tranny shop.
HELP! Any ideas?

Trans new or rebuilt? Did someone put a shift kit in it?
Putting your foot in it should kick it out of 4th?

I had the identical problem with 1st and 2nd gear shift, and your OD shifting issue is most likely the TV cable.
I had my trans rebuilt at a local shop and when I got it back it shifted like new, no more neck snapping 1-2 shifts. So I don't know what they did but whatever they overhauled, it worked.
Had a friend I trust adjust the TV cable and OD shifted as it should.
