Are you talking about using a front end like off a Topkick truck or something like that? If so I don't know what I was looking for but on Youtube one day I saw a guy with a lifted blazer that had one But I can't find it again. I think he was playing in the mud on some kind of trail.

The doors are different on those that standard pickups.

1) yes

2) i'm somewhat aware that the medium duty vs. light trucks had different doors at the very bottom front corner and i'm willing to make adjustments.
