When you take the oil fill cap off my friends 350 smoke puffs out the hole while the engine. The pcv system seems to be in good working order. Would this indicate blow by? The truck has 194k miles on it.

It sound like blowby to me. You can try some good decarbonizer and conditioner to help the rings but if they're worn they're worn.

i had an old 350 that would just blow the pcv valve out when ever it felt like it lol would make a heck of a pop when it blew dew to the pressure it would build up
