Thanks for sharing.  I saved the template.  Some parts stores may carry a similar device made out of hard plastic.  I've seen them before but can't remember where.

Here is a quick reference for 5 bolt pattern wheels.  I have written down the info on a business card and I always have my leatherman to measure with.

Small world.  I live about 1/2 hr drive from the address on that bolt template.
